Forms & Documents Needed for Juvenile Justice Specialist Employment Screening

If you would like to apply for a Juvenile Justice Specialist position, you will need to attend an employment screening. Be sure to go through all information on this page so you are fully prepared for your screening!

  1. Be sure you meet the qualifications listed on the Careers page.
  2.  Fill out an Electronic Application at least 3 business days before the screening you will attend.
  3. Download the CMS 100 form. It must be printed, completed, and brought to the screening. Other forms are listed below, but the CMS 100 is the only one that MUST be completed and brought to screening. 
  4. Gather the needed identification documents listed below. Most importantly, bring a COPY of your driver's license AND your original license to the screening
  5. Attend an employment screening. A current list of screening dates & details can be found on the Careers page.  (Be sure you have done the electronic application a few days in advance. Don't forget to bring your driver's license, a copy of your driver's license, and the printed and completed CMS 100 form. Also bring any of the other additional forms or documents listed below that you have completed.)

Documents to bring to screening

Please bring COPIES of the materials listed below. It's best to bring all of the below, but you must have a copy of your driver's license and the original license to complete the screening. 

  • Driver’s license-you must bring a copy AND the original license
  • COPY of Birth Certificate
  • COPY of high school diploma or college transcripts (if applicable). Unofficial transcript copy is acceptable as long as it states degree awarded.
  • Selective Service number if applicable (Males 26 and under)

 

Completed forms to bring to screening

You must bring the CMS 100 form to your screening, but you are encouraged to download, print, and complete all of the below forms and bring them to the screening (or as many as possible/applicable). This will help expedite the screening and hiring process greatly! 

  1. CMS 100 (This must be completed and brought to employment screening.)

  3. DJJ 0031 Applicant Information Sheet (AIS)       
  4. DJJ 0037 Employee Reference Check (ERC)
  5. DJJ 0035 Employment Release and Consent
  6. Background Check Authorization (CANTS)

  8. CMS Military Verification Letter
